Diabetes and Exercise Consent Form
Invitation and Consent Form
All students between the age of 18 and 64 any students above or below this age range please do not submit form for this study.
Study Purpose
The purpose of this study is to provide students with a survey to assess their current sleeping habits.
Surveys:
All subjects in this study will consist of individuals who are currently enrolled in school, during a week before an exam. After this assessment, follow up with another survey the next week discussing the difficulty level of memorizing and remembering the information studied, along with their final grade. The principal investigator Dr. Hammer will be the only person with access to this data.
Sleep Study:
This study will provide students with a way to assess their current sleeping habits before and after an exam.
Study Procedures
If you agree to participate in this study, we would ask you to do the following: to complete the questionnaire one week prior to an exam. Research will be conducted through a link offered to individuals. After this assessment, a follow up with another survey the next week discussing the difficulty level of memorizing and remembering information studied, as well as the final grade.
Risk of Study Participation
There is no risk factor in this assessment seeing as everything is done online through a computer.
Benefits of Study Participation
Benefits of participating in this study are to gain direct knowledge about how late hours of studying and exam preparation can affect the psychological wellbeing of students.
Research Related Injury
This study will be conducted as a measurement of activity that the participants have agreed to upon their own free will. All testing procedures are non-invasive and have no risk of causing injury. Any injury occurring in the actual study will be unrelated to this data collection.
Confidentiality
The records of this study will be kept private and confidential, to the extent allowed by law. The data from this survey will be used as a “Pilot study” and will not be used for any type of publication. Data from the survey will be only portion of survey used, no one will use names or identifying information.
Protected Health Information (PHI)
Your PHI created or received for the purposes of this study is protected under the federal regulation known as HIPPA. Refer to the HIPPA authorization details concerning the use of this information.
Voluntary Nature of the Study
Participating in this study is voluntary. Your decision whether or not to participate in this study will not affect your current or future relationship with the university. If you decide to participate. You are free to withdraw at any time without affecting those relationships.
